change doctor name and add doctor

Management of the doctors is not included in the project - in order to keep it simple.

1. You can add traditional form-based web interface to edit the doctors table.

2. You can extend the manager's interface to use the built-in Scheduler UI controls:

• Row creating for adding doctors (https://doc.daypilot.org/scheduler/row-creating/)
• Row header context menu or inline row editing for changing the existing doctor records (see https://doc.daypilot.org/scheduler/row-header-context-menu/ and https://doc.daypilot.org/scheduler/row-editing/)
